Two studies about testosterone came out this year, and between them they have produced more confusion than clarity.
The first found that men with genetically higher lifelong testosterone had roughly 17% higher odds of coronary artery disease. The second followed 9,537 men on testosterone therapy and found improvement across every quality-of-life measure it tracked — libido, energy, strength, erection quality.
You have probably seen one of these. Most men have not seen both. And a man who sees only one tends to form a permanent opinion about testosterone based on whichever half happened to cross his feed.
Here is the useful part: both studies are sound, and they are not in conflict. Once you see why, you will also know which number you should have been tracking all along — and it is not your testosterone.
What the first study actually measured
Published in The Journal of Clinical Endocrinology & Metabolism, this was a Mendelian randomization analysis drawing on more than 400,000 adults in UK Biobank and over a million individuals in a cardiac genetics consortium.
Mendelian randomization is worth understanding, because it is a genuinely strong design. You inherit your genes at conception — before your diet, your training, your stress load, your drinking, or forty years of anything else can muddy the water. So if you sort men by the genetic variants that set their testosterone higher or lower, you have something close to a natural experiment. The usual confounders never got a chance to enter.
The finding: men with genetically higher circulating testosterone had higher odds of coronary artery disease — an odds ratio of 1.17, with a 95% confidence interval of 1.07 to 1.27. There was no equivalent effect in women.
And the researchers went further than the association. They identified the likely mechanism, which is the part that matters most to you. The risk appeared to be mediated in part by higher blood pressure.
The hormone was not acting on the heart directly. It was acting through the plumbing.
What the second study actually measured
Published in the World Journal of Men’s Health in April 2026, this was a longitudinal retrospective cohort of 9,537 men treated for testosterone deficiency by a UK men’s-health provider between 2019 and 2024. Nearly 90% were on subcutaneous injections.
The results were meaningful. Improvement across all eight quality-of-life domains, with libido moving from a mean score of 1.94 to 3.20. Hematocrit elevations above 0.54 were uncommon.
That is not a small study and it is not a trivial result. For a man who is genuinely deficient, this is a description of his life getting better.
But read the design, because the paper does not hide it. Retrospective. No control group. Median age 42 — younger than most of the men reading this. And mean follow-up of 8.47 months.
Eight and a half months tells you a great deal about symptoms. It tells you nothing about arteries at sixty-five.
Why these two findings are not in conflict
Line them up side by side and the apparent contradiction dissolves.

Different exposure. Different population. Different timescale. Different question. They were never arguing with each other. Reading them as a contradiction is the only actual error in this story.
The sentence almost nobody quoted
Now the detail that should have been the headline, and which barely appeared in consumer coverage at all.
The same research also ran an observational analysis — looking at men’s measured testosterone rather than their genetics. In that arm, the association ran in the opposite direction. Low testosterone looked like the danger.
The authors explain the reversal. It was confounded by type 2 diabetes and body mass index.
Sit with that for a second. They had to statistically correct for metabolic disease, because metabolic disease was dragging testosterone down and making the raw numbers point the wrong way.
This is the thing this brand has been saying for years, appearing inside a methods section in a major endocrinology journal. Low testosterone is frequently the body reporting on the metabolism — not the origin of the problem.
The mechanisms are not mysterious:
- Most of the day’s testosterone production happens during sleep. Broken sleep means a lower number.
- Visceral fat converts testosterone to estrogen through the aromatase enzyme.
- Insulin resistance interferes with the signal running from the brain to the testicles.
- Chronic alcohol, chronic under-recovery, and several very common medications all land on the same panel.
So the man with the flat energy, the soft libido, the poor sleep and the 38-inch waist frequently has a low number because of that picture. The body keeps score, and the hormone panel is one of the places it writes the score down.
Blood pressure is the number that got buried
Go back to the mechanism in the genetic study. The cardiac risk ran through blood pressure.
That should stop a man in his tracks, because blood pressure is also the most direct readout available of the arterial health behind erectile function. Erections are vascular events. Blood flow is a system issue, not a bedroom issue — and here is a large genetic study demonstrating exactly that, with the hormone reaching the heart by way of the vascular system rather than by acting on it directly.
Which means the practical takeaway is not “avoid testosterone.” It is this: whatever you decide about your hormones, you had better be watching your blood pressure while you do it.
The two ways men get this wrong
The first mistake: reading “17% higher coronary risk” as a treatment risk and avoiding care. It is not a treatment risk. It is a measure of lifelong genetic exposure in the general male population. A man with genuine deficiency who talks himself out of a doctor’s appointment because of that headline has been harmed by bad science communication, not protected by it.
The second mistake, and the more common one: reading 8.5 uncontrolled months as a lifetime safety guarantee, and skipping the workup entirely.
The Endocrine Society published a statement on July 16, 2026 reinforcing that accurate diagnosis and proper clinical assessment must come before testosterone therapy is started, and flagging continuing uncertainty about long-term safety. Data presented at ENDO 2026 found that only a small share of men prescribed testosterone had received the diagnostic testing the guidelines call for.
That statement is not bureaucracy. It is a response to something that is actually happening.
What to do before you make a hormone decision
Measure first
- Get a home blood pressure cuff. Two readings, morning and evening, for seven days. Bring the log to your appointment. This is the exact mechanism the study identified, and almost no man walks into a hormone consultation with this number in hand.
- Waist circumference at the navel. Track it monthly.
The labs to ask for
- Two morning total testosterone draws on separate days. The number swings considerably. One afternoon reading is not a diagnosis.
- Free testosterone and SHBG.
- LH and FSH — these help separate a problem at the factory from a problem with the signal telling the factory to run.
- A1c or fasting insulin, plus a lipid panel.
Food
- Nitrate-rich foods associated with nitric oxide production: beets, arugula, spinach, pomegranate.
- Protein at roughly 1.6 grams per kilogram of bodyweight, to defend lean mass.
- Potassium-forward foods for blood pressure support: beans, potatoes with the skin on, leafy greens.
- Move the alcohol earlier in the evening, or out. It degrades sleep quality on the same night the testosterone is meant to be produced.
Movement
- Resistance training, 90 to 119 minutes per week. A long-term cohort published in the British Journal of Sports Medicine associated that range with 13% lower all-cause mortality and 19% lower cardiovascular mortality. That is about three sessions. It is an achievable number.
- Zone 2 cardio, two to three sessions per week, for arterial function.
Recovery
- Seven to nine hours, at a consistent bedtime.
- If snoring, gasping, or waking unrefreshed is part of your picture, ask about a sleep study before concluding the hormone is where the story starts.
The number was never the target
Get the blood pressure log. Get the four labs. Get the sleep looked at. Then have the hormone conversation with a doctor who wants to see all of it — not a clinic that only wants to see one number.
And be clear about this: if therapy turns out to be right for you, none of that work is wasted. It is the baseline that makes the therapy safe to run, and the only way you would know if something drifted.
Blood pressure. Sleep. Muscle. Glucose control. Those were always the system. The number was only ever the report.
